Various arthritic conditions and several geneticand neuromuscular diseases, such as Down syndrome and Marfan syndrome, cause muscle imbalances that can create bunions from displacement of the first metatarsal which causes big toe pain.
Other possible causes are bunions and leg-length discrepancies, with the bunion present on the longer leg, trauma can occur to the joint of the big toe. Inflammation of this area causes a decrease in motion with associated discomfort in the joint between the big toe and the first metatarsal.
In many people, gout initially affects the joints of the big toe (a condition called toe joint pain). Many other joints and areas around the joints can be affected in addition to the big toe. The big toe is forced in toward the rest of the toes, causing the head of the first metatarsal bone to jut out and rub against the side of your shoe; then the underlying tissue becomes inflamed and then a painful bump forms. The Sesamoiditis is an inflammation of the tendons around the small, round bones that are imbedded in the head of the first metatarsal bone, which leads to the big toe pain.
